Integration # 1

I just finished listening to all of #4. I have a rather urgent question. The video at the end stopped by offering me a book for $139.95 plus tax.
If this book is as advertised, I want it! However, there were no instructions as to how & where to send it. Can you give me this information? Or tell me how to get it.
I enjoyed level 4 and do have some questions, but I would like to send them after reviewing my notes. I will appreciate any advice you can give.
